COVID-19 OFFICE POLICIES
Our waiting room is now open and you may come right up to the office to check in. Masks are no longer mandatory, but we do recommend you continue to use a mask if you do not feel well.
1. You MUST complete all patient forms on the Patient Portal prior to the visit. Patients who do not complete the online forms will not be able to be seen. Paper forms are not available.
2. Please have your payment type ready upon arrival to the office. If you are a new patient, also have your driver's license and insurance cards ready.
